Program Purpose
To ensure each residentโs physical, mental, and emotional well-being through a structured, nurse-supervised wellness approach that complies with Title 22, Division 6, Chapter 8 of the California Code of Regulations.

At Oak Canyon Living, we believe wellness thrives in a home environment where compassion, dignity, and respect guide every interaction. Under the direction of Jerica Gonzales, LVN, our program integrates professional nursing insight with personalized, family-style support.
Scope of Services
Our Health & Wellness Program promotes well-being through:
- Daily health and safety observations by trained staff under LVN supervision
- Coordination with physicians, home health, and hospice services
- Regular weight, appetite, and hydration monitoring
- Medication assistance per Title 22 guidelines (self-administration support only)
- Preventive wellness activities (mobility, hydration, rest, nutrition)
- Education and refreshers for caregivers on infection control, fall prevention, and residentsโ personal preferences
Monitoring & Documentation
To maintain quality of care, Oak Canyon Living implements:
- Health Observation Notes for changes in condition
- Monthly Wellness Summaries reviewed by the LVN
- Medication Assistance Logs in compliance with ยง87465
- Incident Reports and Follow-Up Documentation for family and physician communication
Coordination of Care
The LVN Health Coordinator works closely with:
- Physicians and pharmacies for accurate medication and treatment communication
- Home health and hospice providers for continuity of care
- Family members to ensure understanding of each residentโs changing needs
Resident Rights & Family Participation
Residents and families are encouraged to participate in planning care routines and wellness activities. Feedback and open communication are welcomed to promote a true sense of partnership in care.
Staff Training
Staff receive ongoing in-service education directed by the LVN, including:
- Recognizing changes in condition
- Infection prevention and control
- Emergency procedures
- Proper documentation practices
Program Evaluation
The Health & Wellness Program is reviewed annually by the LVN to assess effectiveness, compliance, and resident satisfaction. Adjustments are made based on residentsโ evolving needs and regulatory updates.

Jasper
Therapy Coordinator
Jasper is our facilities therapy coordinator. He is available for counseling and therapy sessions that are just pawsome! His duties include looking cute to reduce mental stress in residents and showing how to live in the moment.
